A quick and simple way to disburse death benefits of $7,500 or less.
Exceptions:   contestable claims;  foreign deaths;  trust or estate as beneficiary; PO Box
addresses;  situations of divorce, suicide or homicide.
Step 1 Notification
Before submitting any claim forms you must either:
     a)  Report the death with our quick and easy Notification Form  OR
     b)  Contact John Hancock at 1-800-387-2747 to report the claim (and to verify the
          beneficiary if you are unsure of the correct beneficiary)
Step 2 Contact the Beneficiary or Funeral Home
Contact the beneficiary or Funeral Home to request:
    a)  Certified copy of a death certificate
    b)  The beneficiary's Tax Identification Number   
NOTE:  These may be faxed to the Claims Department at (416) 926-3100.
Step 3 Await Completion of Claims Process
Await completion of the claim process, which is performed in a timely manner.
Step 4 Deliver Proceeds
Deliver the death benefit proceeds, where appropriate.
